The Delta State Government has formally notified the public of the death of a Deputy Director in the Ministry of Basic and Secondary Education, Mr. Adeda Ekwevugbe, amidst unclear circumstances in his workplace.
In a statement issued by the New Media Office of the Governor, the government said the incident has provoked serious concerns among colleagues at work and vast sections of the public service, including Organised Labour and Government.
According to the statement, while the Police has commenced investigation into the incident, government has consulted with Organised Labour and jointly agreed to constitute an Administrative Panel of Inquiry into the incident.
The eight-member panel, which is expected to submit its findings within two weeks, is constituted as follows:
I. Mrs. Charity Ehimen – Chairman
Permanent Secretary, Office of the Head of Service
II. Mr. Mamuzo Erebe, SAN – Member
Solicitor General/Permanent Secretary, Ministry of Justice
III. Mr. Lucky Nkemachor – Member
Permanent Secretary, Office of the Secretary to the State Government
IV. Sir A.E Oghoro – Member, among others.
The government commiserated with the family of the deceased and reaffirmed its commitment to the safety of civil servants and protection of the dignity and sanctity of life.
The panel is to work alongside the ongoing Police investigation.
